4. PPD/TB Skin test/X-ray
5. CPR certification
6. Void Check
7. Background check - We will send it to you once we receive the completed application. You will sign, have it notarized with your bank, and scan it back to us.
8. Professional Liability Insurance - The agency will pay for it
